Helpful Tips

- To download a paper-copy of the form, press the button “Question List”.
- Required fields are marked with an asterisk (*).
- Each response must stay within the designated character limit. If any responses exceed the limit, the system will prevent submission.
- If an attachment exceeds the file size limit, email the attachment to your FMH Staff contact. Upload a brief Word document indicating that you have submitted the attachment by email.
- To download a PDF copy of your full application, select the “Application Packet”.
- After completing your application, click “Submit Application.” Once submitted, the application cannot be edited. If you need to make changes, contact Foundation staff to request a reversal.
- You will receive an automated email confirming your submission. If you do not receive confirmation or are unsure whether your submission was successful, please call the FMH Foundation office at 432-682-7924.
- If you are awarded a grant, required reports are due by 4:00 p.m. on the following dates:
- January 20th
- April 20th
- July 20th and/or
- October 20th
- After each report is submitted, you will receive a confirmation message.
- Timely submission of all required reports is an important factor in determining eligibility for future funding.
